X-Rays

One of the key components of Chiropractic care is diagnostics. Chiropractic radiology serves as a critical tool in this process, aiding in the identification of potential pathologies such as tumors or fractures, as well as guiding spinal adjustments. Chiropractors utilize a variety of diagnostic techniques to gain a comprehensive understanding of a patient’s neuromusculoskeletal system and assess the most effective treatment interventions for achieving optimal results.

Given that Chiropractic care often involves spinal manipulation and other techniques that require the movement of skeletal structures, X-rays provide valuable insight into the alignment and structural integrity of the body.

It is important to note that X-rays are routinely used in Chiropractic care. For the majority of patients, Chiropractors can assess musculoskeletal conditions through physical examinations and confirm their findings using X-ray imaging. X-rays are typically for severe condition, where there may be underlying conditions such as scoliosis, osteoporosis, or spinal stenosis.

In general, X-rays are recommended in the following circumstances:

Following significant traumatic injury, where fractures or dislocations may be present
When an infection may be contributing to the patient’s pain
If there is suspicion of a serious disease, such as cancer or a tumor
When joint disease, such as arthritis, is suspected to be causing pain
For patients over the age of 50 who have experienced any form of trauma (even minor)
For most patients aged 65 and older
For those diagnosed with or at risk for osteoporosis, to identify or rule out vertebral fractures
In cases of suspected spinal instability
For patients experiencing long-standing pain that has not responded to previous treatments

X-rays (or diagnostic images) play a pivotal role in Chiropractic care by enabling Chiropractors to visualize bones, joints, and related soft tissues, with the most clarity being in the bones and joints. When taken in a standing or weight-bearing position, X-rays reveal both normal and abnormal spinal curvatures, as well as joint changes. This allows Chiropractors to evaluate bone health, identify congenital anomalies, fractures, dislocations, arthritis, infections, and tumors, all of which significantly influence the treatment approach.

Historically, X-rays were commonly used for Chiropractic patients; however, advancements in research and education have led to a more selective approach. Chiropractors now rely on a thorough physical examination and the patient’s medical history to determine whether imaging is necessary. While X-rays are often taken in cases of serious trauma or a cancer history, the decision ultimately depends on the clinical assessment.

X-rays are more frequently utilized for older patients, as they are more likely to have conditions such as brittle bone disease, which could alter treatment strategies.

What Patients Want to Know Before an Imaging Exam

Concerns about radiation exposure are common among patients undergoing spinal imaging exams. A study published in Radiology (February 2018) examined what information patients and caregivers prioritize before undergoing imaging procedures and how they prefer to receive it.

The study found that patient care in radiology tends to focus on test results post-exam, but pre-exam communication is equally important. Notably, 22% of participants reported receiving no information prior to their imaging exam.

Types of Spinal Imaging Exams

Different imaging tests are available to diagnose and treat spinal conditions. A common misconception is that all imaging tests involve radiation; however, X-rays, CT scans, and nuclear medicine studies do emit radiation, whereas MRIs and musculoskeletal sonography do not.

Study Findings on Patient Preferences

The research involved a survey of 1,542 patients and caregivers, revealing the following key findings:

The majority (72%) preferred to receive information from the doctor who ordered the exam, followed by radiology staff (21%).
Most patients (52%) preferred to receive information about the exam via phone, while 37% preferred in-person discussions.
Over half (52%) took the initiative to seek information about their exams through direct contact with their provider or online searches.

When it came to the types of information that patients found most valuable, respondents prioritized practical details such as:

How to prepare for the exam
The process of the exam itself
Whether an intravenous (IV) line or drinking contrast material would be necessary

Conversely, less importance was placed on radiation-related concerns, such as the amount of radiation used or alternatives that do not involve radiation.

The study highlighted a disconnect between public concerns about radiation exposure and the reality of patient preferences, suggesting that anxiety about the procedure and its preparation were more pressing concerns for most patients than radiation risks. This underscores the need for clearer communication and more education about the safety and necessity of imaging exams, particularly those involving radiation.

Why X-rays?

Rather than lying down, we want X-ray views showing your body in a weight-bearing position.

Both visible light and X-rays consist of photon particles conveyed over electromagnetic energy. When carefully controlled, this energy can be used to reveal the internal structure of the body. We find these pictures quite helpful:

Provide a structural record of your spine

Rule out cancer, disease or other pathologies
Locate fractures or other signs of trauma
Identify evidence of long term spinal decay
Confirm other examination findings

Interestingly, X-rays were discovered soon after the first chiropractic adjustment in 1895. Wilhelm Roentgen, a German physics professor, was experimenting with cathode ray tubes and discovered that an object across the room began to glow. Much to his surprise, when he placed his hand between the tube and the glowing barium screen, he was able to see the bones of his hand.

From those crude beginnings over a century ago, both chiropractic and X-ray imaging have come a long way. Today, the chiropractic profession is a leader in the reduction of X-ray exposure and patient safety.

TWO Facts Every Patient Should Know About X-Rays

Commonly, people living with scoliosis dread x-rays. After seeing their condition steadily deteriorate with x-ray each series, combined with the fear of too much radiation, some patients are reluctant to have more x-rays taken. But before you hesitate to undergo alternative scoliosis treatment for this reason, please consider the following facts about scoliosis x-rays.

According to a study published in August of 2000 in the Journal of Radiology, “Patients undergoing scoliosis radiography receive doses that are low in comparison with other types of radiographic examination.” A similar study published that same month in Spine found that patients with scoliosis that received fifty or more x-rays from 1912-1965 had a risk of dying from breast cancer that was four times higher than non-x-rayed patients. At that time, there was a need for caution, since standard films were taken at 0.6 cGy (centigray). Today, films are shot with only 0.02 cGy – thirty times less than in the past. To achieve the same level of exposure, over 1500 x-rays would have to be taken on one patient. Pediatric x-rays are shot at an even lower level of magnitude.
When taking a precision x-ray – that is, one not to rule out fractures or abnormalities, but rather to measure the position of the spinal units – positioning matters more than one might think. Even a slight rotation off from center will skew the results and show an inaccurate image. For this reason, we must be very concerned about how the x-rays were taken. They must allow an objective outcome assessment. X-rays from other sources oftentimes can be unreliable if they are shot at different angles or focal distances, which prevent comparison between films. For this reason, a special device is used to assure accurate findings.
X-rays allow us to measure and quantify the results of treatment, and determine if alterations to existing treatment protocols are necessary. If you are seeking chiropractic care for your scoliosis, you should insist upon x-rays that are accurate and scientific; follow-up films should be taken every three months, and always in the exact same positioning.

Spinal Decay and X-rays

Soon after the discovery of the X-ray, chiropractors began using this tool in the examination and care of the spine. Besides revealing the presence or absence of proper spinal curves, side views of the spine can more easily show the long-term effects of neglected spinal trauma. We call the body’s adaptation process spinal decay.

Why Spinal Decay?

This process in the spine has similarities to tooth decay in that it shows up usually because of neglect. Also, just like tooth decay, it can happen to anyone, regardless of age. Because it is often seen in older people, many believe it’s merely the normal aging process. It is not. This progressively worsening, degenerative condition is the result of uncorrected spinal misalignment. Spinal decay is how your body deals with the stress of gravity. It just so happens that most people have spinal problems that they’ve ignored for years.

Three Phase Process

Spinal decay is a process. Whether you take three snapshots of the process or twenty-three snapshots, it’s a process. We use the three-phase model based on the book Managing Low Back Pain, by W. H. Kirkaldly-Willis and Thomas Bernard, Jr. We find that it makes the differences between each phase great enough so our patients can appreciate what they’re seeing.

While this process can occur anywhere in the spine, it’s easiest to see in the cervical (neck) spine.

This is what the side view of the neck should look like. From the side, the neck should have

a graceful forward curve.

The edges of each bone are well defined.

There should be equal disc spacing between each bone and the openings for nerve roots should be clear and unobstructed.

This would be considered a textbook “normal” side view of the neck.

Phase One

Phase One Spinal Decay. The first thing you notice is a loss of the normal cervical curve.

In Phase One there is a loss of curve and a reduced ability to turn and bend. In some cases, the disc space between adjacent vertebrae may be reduced. Or, discs canbecome wedge-shaped risking more serious soft tissue damage Depending upon your body’s adaptive capacity, pain or other obvious symptoms may not be present. Even the resulting reduced range of motion may be too subtle for some to notice.

Phase Two

Phase Two Spinal Decay. Adjacent joint surfaces become rough and uneven.

If neglected, in Phase Two, joint surfaces lose their clearly defined edges as the early signs of bone spurs become visible on X-ray views.

Some believe that the body senses the accompany- ing inflammation as the sign of a “broken bone”. It begins the process of depositing calcium salts. Without being in a cast to prevent movement, the fusion doesn’t occur. Yet!

Phase Three

Phase Three Spinal Decay. The affected bones have lost their individuality & ability to move.

If still left uncorrected, the problem worsens in Phase Three. Amazingly, pain or other obvious symptoms may not yet show up. But finally, the body successfully “mends the broken bone.” This process can take years. Its presence calls into question the claim that many patients make that they were “…fine until last week when I bent over to tie my shoes.” That’s not what the X-rays tell us!

The Prognosis

Some patients who begin chiropractic care in Phase One discover a restoration of their spinal curves and improved range of motion.

Success with Phase Two patients is less predictable and depends on many factors. While we can do little after decades of neglect with Phase Three cases, we instead turn our attention to those areas of the spine that have not yet been lost to the process.

It’s impossible to predict what effect, if any, chiropractic care will have on slowing, stopping or reversing this process. Nor can a particular phase be used to predict how much care will be needed.
